How to Get Started

Remember that by Iowa Law, all children to be enrolled in Kindergarten for the 2017-2018 school year must be age 5 by September 15, 2017.

1 FIND YOUR SCHOOL
Not sure what school you attend? Determine your home school at www.dbqschools.org/find-your-school or call 563/552-3275. Even if you wish to apply for open enrollment, you should start the process at your home school.
2 START THE SIGN-UP PROCESS (bring the following to your home school)
New Student Enrollment Packet (PDF)
Medical History Questionnaire (PDF)
Proof-of-Age (birth certificate)
Most Recent Immunization Record to Date (this record may need to be updated as your child receives new immunizations)
3 ATTEND YOUR SCHOOL’S WELCOME EVENT

4 ADDITIONAL REQUIRED FORMS (must be completed and returned by the first day of school)
Certificate of Immunization (PDF) (children will not be allowed to attend school without this submitted; to be completed by a healthcare provider)
Certificate of Dental Screening (PDF) (to be completed by a dental care provider)
Medical Examination Form (PDF) (to be completed by a healthcare provider)
Request for Medication to be Given at School Form (PDF) (needed only if your child requires medication to be administered at school; to be completed by a healthcare provider)

For Your Information

> The Iowa Department of Public Health requires a vision screening no sooner than one year before
the beginning of kindergarten and no later than six months after the start of school. Kindergarten students in the Dubuque Community School District are provided a vision screening at school in the fall, which fulfills this requirement. If you wish to have a screening by another provider, you can download and print the Certificate of Vision Screening (PDF).
> Do you know when your child is too sick to go to school? “Is my child too ill to attend school?” is a handy resource to know when to attend school and when to stay home.
> A lead test is required by the State of Iowa. Your school will request a “yes or no” answer from a parent when inquiring about whether or not the child has had a lead test (if not indicated on the Medical Examination Form). The school then submits a list to the Iowa Department of Public Health and will follow-up with families to ensure lead testing occurs if needed.

When do you actually register?

You will receive a registration packet from the district in late summer with instructions on how to officially register through PowerSchool, the district’s online student information system.

Prescott Elementary School is an Expeditionary Learning charter school open to any child who resides in the Dubuque Community School District, with a limited number of kindergarten openings. The 2017-2018 enrollment period for Prescott runs from February 27-March 10. To register at Prescott, you must contact the school IN PERSON during this time period.


Families wishing to have a student attend kindergarten at a school other than their home school may apply for open enrollment between March 15 and May 1. For more information or to obtain the necessary forms, visit www.dbqschools.org/open-enrollment.
