For Your Information
- The Iowa Department of Public Health requires a vision screening no sooner than one year before the beginning of kindergarten and no later than six months after the start of school. Kindergarten students in the Dubuque Community School District are provided a vision screening at school in the fall, which fulfills this requirement. If you wish to have a screening by another provider, download and print the Certificate of Vision Screening (PDF) or ask your eye care provider.
- A lead test is required by the State of Iowa. The school submits a list to the Iowa Department of Public Health and follows-up with families to ensure lead testing occurs if needed.
- If your child requires medication to be administered at school, the Request for Medication to be Given at School Form is available in your school’s nurses office.
When do you actually register?
You will receive a registration packet from the district in late summer with instructions on how to officially register through the district’s online student information system.
